Prenatal Plus is a Medicaid-funded program that provides services to Medicaid-eligible pregnant women in Colorado who are at high risk for delivering low birth-weight infants. Prenatal Plus services include comprehensive risk assessments, care coordination, home visits, nutrition consultation and counseling.
